We have created this page for our clients to help them prepare when visiting our veterinary hospital for important procedures. Our drop-off forms are available here. You can download these forms and fill it out prior to drop-off to make surgery day as easy and stress-free as possible. It is imperative that you carefully read over all the information and initial in case you do not wish to receive pre-anesthetic bloodwork (not optional for some procedures) and then sign the bottom.

Please ensure the phone number written on the form is available for contact during surgery if we need to have any medical decisions made while your pet is under anesthesia, or in case of an emergency. If we are not able to reach you at the phone number provided by you, the doctor may need to make the decision which is best for the pet in that situation and extra fees may apply.”
